Cumberland Pharmaceuticals Inc. announced plans to submit a New Drug Application to the U.S. Food and Drug Administration for ifetroban as a treatment for cardiomyopathy associated with Duchenne muscular dystrophy. The company disclosed this development in a Form 8-K filed on October 26, 2026, following a pre-NDA meeting held with the FDA this fall.
The application will draw on evidence from the FIGHT DMD clinical trial and its open-label extension, alongside data from two natural history databases of DMD patients. It will also incorporate preclinical findings published in the Journal of the American Heart Association, manufacturing details, and a safety record spanning more than 1,400 subjects across roughly 30 ifetroban studies. Cumberland intends to use the FIGHT DMD trial as the pivotal study, supported by comparisons from the two natural history databases.
In the 12-month Phase 2 FIGHT DMD trial, high-dose ifetroban produced a statistically significant 5.4% absolute improvement in left ventricular ejection fraction compared to a control group of placebo-treated and propensity score-matched natural history patients. The high-dose group also showed reductions in cardiac damage markers, specifically NT-proBNP and cardiac troponin I. All patients who finished the 12-month study chose to continue in the open-label extension, with cardiac MRI data from Months 24 and 36 currently under analysis for inclusion in the NDA.
The decision to pursue Traditional Approval followed an End-of-Phase 2 meeting with the FDA in September 2025 and a Type C meeting in January 2026. During those interactions, the FDA suggested a Phase 3, randomized, placebo-controlled trial and questioned the LVEF endpoint, comparator issues, and statistical analysis plan. Cumberland responded by providing additional efficacy data, natural history data from a second DMD registry, and documentation of the statistical analysis plan's development.
At the pre-NDA meeting this fall, the FDA raised concerns about whether the proposed data package met the requirement for substantial evidence of efficacy and requested further analysis. The agency granted Cumberland's request for rolling review, allowing completed sections to be submitted before the full application. The company's Board of Directors recently approved proceeding with the NDA submission to address the unmet medical need in this patient population.
Ifetroban is a once-daily oral medication that blocks the thromboxane-prostanoid receptor. It holds Orphan Drug Designation, Rare Pediatric Disease Designation, and Fast Track Designation from the FDA for this indication. Exploratory analyses showed a 30% reduction in MYL3 and a 50% reduction in MYOD1 with 12 months of high-dose treatment compared to the control group. Cumberland stated there can be no assurance that the FDA will accept the NDA for filing or approve ifetroban based on the submitted data.
